What is Diabetes mellitus? What are the types?

Diabetes mellitus is a chronic disease where the body’s capability to produce or respond to insulin hormone is affected due to which the metabolism of carbohydrates does not happen properly resulting in raised blood glucose levels. High glucose levels in the blood cause serious health problems like heart disease, vision loss, circulation issues, neurological problems, and kidney disease in due course

Diabetes is usually found in 4 types

  1. Prediabetes: Blood sugar is higher yet not in the range of being diagnosed as Diabetes mellitus. More than a third of the adult population is estimated to be in this category. The condition, if not managed properly may progress to Type 1 Diabetes
  2. Type 1 Diabetes: Commonly referred to as insulin-dependent diabetes and juvenile-onset diabetes, the disease usually starts in childhood and is an autoimmune condition. The pancreas is damaged and hence insulin production is impaired. All complications of Diabetes mellitus can happen with more risk of heart disease and stroke
  3. Type 2 Diabetes: Commonly referred to as non-insulin-dependent or adult-onset diabetes. More than About 90% of people with diabetes come under this category. Insulin production is mostly present but not enough for the required metabolism. Insulin resistance, a condition of cells not responding to insulin is usually common in fat, liver, and muscle cells. Obesity and a sedentary lifestyle increase the risk and complications
  4. Gestational Diabetes: Insulin resistance during pregnancy causes gestational diabetes. Control of the condition is important to safeguard fetal growth and development. Risks to babies include overweight baby, breathing problems immediately after birth, higher risk of obesity and diabetes later in life

What are the causes and risk factors of Diabetes?

  • Type 1: Causes are hypothesized as a combination of genetic susceptibility and environmental factors that affect the immune response
  • Type 2: Heredity usually maternal, obesity, sedentary lifestyle, wrong diet, high intake of high-fat food, metabolic disturbances, disease conditions, medicines like steroids, and environmental factors.
  • Gestational Diabetes: Excessive hormone production by the placenta to sustain the pregnancy leading to insulin resistance

What are the Signs and Symptoms of Diabetes mellitus?

  • 3 Ps: Polyuria (Excessive Urine), Polyphagia (Excessive Hunger), Polydipsia (Excessive Thirst)
  • Increased thirst and extreme hunger
  • Sudden unexplained weight loss
  • Frequent urination
  • Presence of ketones in the urine
  • Fatigue and Irritability
  • Blurred vision
  • Slow-healing sores
  • Recurrent infections

  • Fasting Blood Sugar Tests: Normal: 90-110 mg/dL Prediabetic: 110-125 mg/dL Diabetic: >126 mg/dL
  • Post-Prandial Blood Sugar Tests: Normal: < 140 mg/dL Prediabetic: 140-180mg/dL Diabetic: >180 mg/dL
  • Glucose tolerance tests: Normal: 140 mg/dL Prediabetic: 140-199 mg/dL Diabetic: > 200 mg/dL

What is the Ayurveda concept of Diabetes mellitus? What are the Ayurveda treatment options for Diabetes mellitus? 

Diabetes is called Prameha and is grouped as one of the eight most morbid diseases or Asthamahagada in Ayurveda. 20 types of Pramehas are described in detail in the Ayurveda texts including Charaka Samhita. Type 2 Diabetes mellitus is Madhumeha. The symptoms are frequent urination with astringent, sweet honey texture and attracting ants, turbidity in urine, obesity, excessive thirst, excessive sweating with foul body odor, breathlessness, pallor, debility, loss of lubrication, loss of libido, etc.

Diabetes is considered to be a Yapyavyadhi, which means an incurable chronic condition that can only be managed to prevent complications. Ayurveda treatment for diabetes is based on the patient’s health condition and constitution. Two treatment protocols are,

  1. Apatarpana: for the obese category and includes internal medications, weight-reducing nutritious diet, exercises, fasting, and cleansing Panchakarma therapies
  2. Santarpana: for chronic, emaciated patients with low immunity and stamina. This includes internal medications, nourishing diet, immune building exercises, Abhyanga, Anuvasana Vasthi, and therapies like Navarakizhi

Which are the Ayurveda herbs useful in Diabetes?

Herbs with bitter and astringent tastes that help in fat and carbohydrate metabolism are commonly used. Self-medication is never to be followed. Some of the medicines are given below. Nisha, Kataka, Methika, Triphala, Shilajatu, Tuvaraka, Makshika, Guduchi,  Meshashringi, Shatavari, Amalaki, and Karavellaka are a few useful herbs.

What Ayurveda diet is helpful in Diabetes?

Timely food intake is important. Low carb diet with low glycaemic index cereals and millets like Yava-Barley, Sambha wheat, Ragi, Jowar, Fox Tail Millet, and Kodo Millet is good. Pulses to be used are red gram, horse gram, and green gram. Vegetables ideal are moringa leaves, bitter gourd, gourd vegetables, banana flower, spinach, fenugreek leaves, etc. Fruits good are oranges half ripe papaya, half-ripe guava, watermelon, apple, Feronia limonia, and gooseberry. Special edible oils extracted from Nikumbha (Danti- Baliospernum montanum), Ingudi, (Balanitis aegyptiaca), Atasi (Linum usitatisimum), and Sarsapa (Mustard) are advised in diabetes

What are the exercises, Yoga, and lifestyle for diabetes?  How do you manage Diabetes mellitus?

Exercises: High endurance sports like wrestling, running, warrior activities like swimming, riding an elephant, horse, cart, or intensive jogging are advised in Ayurveda. Contemporary similar activities such as swimming, treadmills, cycling, jogging, etc. can be followed

Ayurveda Home Remedy for Headache

Minor changes in the life routine can be the cause of minor health issues spurting from the changes in physiology. Most of these get corrected on their own while others can be corrected with simple home remedies. Any ailment that does not get corrected within half a day of taking a home remedy requires medical attention. It is to be noted that a home remedy is not a complete treatment instead it is a means of complementing the body’s self-healing mechanism. It is important to know the type of headache. Cold, Sinusitis, Migraine, Tension Headache, and Cluster Headaches are the most common headaches out of which cluster headaches are the commonest

What are the simple home remedies for headaches due to cold?

  1. Add to your moisturiser a few drops of any volatile essential oil like Karpooradi Thailam or clove oil or lemon grass oil or any other simple essential oil that can reduce pain, inflammation, and congestion. Mint oil, Eucalyptus oil, Lavender oil, Teatree oil, Rosemary oil, Sambrani (Frankincense oil), Chamomile oil, etc. are a few suggestions. Apply this mixture to the forehead
  2. Boil water with select herbs as available and advised by your doctor and take steam inhalation. Any aromatic or intense-smelling herbs or vegetables may be used for the purpose
  3. Warm a soft cloth on any hot surface and place it on the painful part of the head and the congested nose to get rid of the headache and congestion due to a blocked nose and sinusitis
  4. Mix Rasnadi Choornam mixed with lime juice and apply it on the forehead. Rest for half an hour with eyes closed
  5. Prepare Chukkukaappi and sip it hot for cold-induced headaches. Cover boil crushed available spices ginger, pepper, cumin, cloves, cinnamon, coriander, turmeric, etc. with a small piece of jaggery for 7-10 minutes. Add a few crushed leaves of Tulasi or Rosemary

What are the home remedies for tension and stress headaches?

  1. For stress- related headaches apply sandal paste over the forehead and rest for half an hour
  2. Mix rose water with milk and apply on to the forehead
  3. Make a paste of home-grown rose petals with milk and apply it to the forehead. Any calming petals may be used for the purpose. Make sure they are not sprayed with chemicals
  4. Apply Kachoradi Choornam mixed with water or milk on the forehead
  5. Prepare a warm drink with crushed coriander seeds boiled in water and sip slowly

What are the home remedies for migraine headaches?

  1. Dim the lights and rest in a calm soothing room preferably with music
  2. Drink enough water at least 3 liters per day
  3. Never skip meals and avoid sugary or spicy fast food in meals. Go for bland soft semi-solid food
  4. Practice Pranayama as prescribed by your clinical yoga doctor
  5. Include food with magnesium like bananas, pumpkin seeds, ash gourd seeds, flax seeds, etc.
  6. Massage the scalp with a few drops of virgin coconut oil

What are the symptoms of common cold?

Change of weather always brings in attacks of cold with it. Viral cold attacks are very common these days. Common symptoms include itching of the nose tip, sneezing, heavy-headedness, blockage of ears and nose,  watering of the eyes, sore throat, hoarse voice, headache, mild fever, fatigue, loss of appetite, and disturbed sleep. Few occasions present with an earache, toothache, etc. It is always to understand the symptoms and manage the situation with simple home remedies and proper rest

When to seek medical attention for the common cold?

Monitoring is required to see that the symptoms do not progress. If they do so medical advice and care have to be considered. Over medication for a simple cold is better avoided. The three principles to take care of cold is to have the right food, stay warm, and take adequate rest. Extreme symptoms of cold are severe throat pain, severe ear ache, splitting headache, high fever, continuous cough, and extreme fatigue. These are to be attended immediately by a doctor to avoid complications like pneumonia

How to take steam inhalation?

For ages cold has been managed through steam inhalation. Plain steam and herb infused fortified steams are used to treat cold. The most efficient and user-friendly steam inhalation device is a wide-mouthed common kitchen vessel with boiling water in it. Steam is inhaled by covering the head to retain the steam inside. Inhaling through the mouth and exhaling through the nose and vice versa is the best means to take steam inhalation. Take steam till you break out into sweat all over the neck and face. It is always good to have intermittent gaps between bouts of inhalation. This eases the strain on the neck and also prevents the facial skin from getting irritated. What to apply before and after steam inhalation?

Apply any cold balm with mint, lemon grass oil, or camphor on the forehead and warm sesame oil on the face before steam inhalation. Apply the balm and massage the forehead, above the eyebrows, above the nose, on the nose bridge, and the ala of the nose gently. In other areas of the face sesame oil or any cream may be used as a precaution for a possible burn and dehydration. Once you break out into sweat dab the face clean, sip a cup of warm water, and relax. While you sit with your eyes closed a herbal pack of turmeric and coriander powder mixed with warm lemon juice can be applied on the forehead for half an hour to get better results.

What herbs can be used for steam inhalation?

Tulsi leaves, (holy basil Ocimum sanctum), crushed black pepper, radish leaves, mint leaves, rosemary, eucalyptus leaves, cedar leaves, pine leaves, betel leaves, crushed ajwain seeds (betel leaf Piper betel) or a handful of diced cabbage may be used for steaming. Singularly or together. Any herb with a volatile oil is suitable for steam inhalation. Locally available fresh herbs like chrysanthemum, etc. can also be tried for steaming

What Ayurveda medicines are useful for steam inhalation?

Rasnadi Choornam, Karpooradi Choornam, Panchagandha Choornam, Trikatu Choornam, Karutha Vattu, Halin drops, etc. may be used for the purpose only with advice and prescription from an Ayurveda doctor

How does steam inhalation work?

The moist, warm steam helps to ease irritation along the respiratory tract right from the nose to the lungs. It reduces the swelling in the blood vessels and mucus membranes in the nasal passages. The moisture also helps to loosen and thin the mucus accumulated in the sinuses and enables easy drainage of mucus. The result is decongestion and easy breathing

What care should be taken while having steam inhalation?

Be careful as you are handling boiling water and you may scald to various degrees. Do not steam after a meal. Do not steam while you are very hungry or sleepy. Consult your doctor if you have diseases like diabetes, heart ailments, hormonal issues, vertigo, etc.

What is Karkitaka Kanji or Monsoon medicated porridge of Kerala Ayurveda?

Karkitaka Kanji is the medicated porridge widely popular in Kerala as part of Karkitaka Chikitsa or Monsoon Regimen practised as per Kerala Ayurveda traditional health care. The porridge is usually prepared with Navara rice, a special variety of rice, an assorted set of herbal powders and spices, coconut or cow or goat milk, cow ghee, jaggery and rock salt. Though there are few regional variations to the porridge recipe, the core principle and ingredients remain the same. Karkitaka Kanji is an adaptation of Kerala Ayurveda aligning with the regional tropical climate and socio-cultural fabric of the society

What are the benefits of Karkitaka Kanji?

Karkitakam is the Malayalam month, the first day of it which falls on 17th of July marks the transition from the dehydrating debilitating harsh Uttarayana to the nourishing calming strengthening Dakshinayana. The settled rains, cleaner water bodies, potent mature herbs and a cooler atmosphere make the season perfect for a holistic body-mind-spirit rejuvenation. The chilling winter, harsh summer the thunderous heavy down pour, the polluted water, surge in vector borne communicable diseases, and many other similar factors batter the health of the population and play havoc with the metabolism during the Uttarayana period between 14th January to 16th July. The imbalance of the Tridoshas especially the Vata Dosha has to be corrected to ensure health for the whole year ahead which is physically and mentally.

What are the ingredients of Karkitaka Kanji?

Ingredients (for 2 persons):

Base:

  • Navara Rice: 50g (4 dessert spoons)
  • Broken raw rice/Broken red rice/Yavam diabetic rice: 50g (4 dessert spoons)

Herbal Powders (available with most reputed Kerala Ayurveda pharmacies)

  • Chandrashoora: Garden cress: 2 teaspoons
  • Dashamoola Choornam: 1 teaspoon
  • Trikatu Choornam: 1 teaspoon
  • Dashapushpa Choornam: 2 teaspoons

Spices (roasted and coarsely powdered)

  • Methika-Fenugreek: 2 teaspoons
  • Jeeraka-Cumin: 2 teaspoons

Taste Maker

  • Jaggery: 3 teaspoons powder
  • Rock salt: Quantity sufficient
  • Cow ghee: 1 teaspoon

Cooking Medium

  • Water: Quantity sufficient

Diluting Additive

  • Coconut milk: Quantity sufficient

How is Karkitaka Kanji prepared?

  • Wash and drain the rice
  • Boil half litre of water
  • Heat a thick bottomed pan and add ¼ teaspoon of cow ghee
  • Add the drained rice and roast the on a low flame for 5- minutes
  • Add boiling water, rock salt to taste and the herbal powders
  • Cover and cook on moderate flame for 15-20 minutes
  • When the rice is soft, add powdered jaggery or jaggery syrup
  • Cook on low flame for 10 minutes
  • Keep stirring
  • When the consistency is soupy, thick and well blended switch off the flame
  • Add the crushed spices and stir
  • Add remaining ghee and stir
  • Add coconut milk and stir for 5 minutes
  • Take care to stir so as to avoid curdling of coconut milk

What are the accompaniments of Karkitaka Kanji?

  • Pathila Thoran or any seasonal sautéed greens
  • Chutney made by crushing ginger, garlic, shallots and bird’s eye chilli garnished with lime juice and rock salt
  • Medicated warm water spiced with tulasi, cinnamon and cloves

How is Karkitaka Kanji served?

  • Serve hot
  • To be taken as breakfast or dinner
  • Serve in a terracotta or brass bowl
  • Use a raw jackfruit leaf spoon or a copper spoon
  • Sit on floor or chair cross legged (Sukhasana) while eating the Karkitaka Kanji
  • Avoid talking or use of gadgets
  • Eat slowly

What are Varicose Veins?

Varicose veins are defined as the superficial veins in the legs that are abnormally swollen or dilated or twisted or enlarged veins due to extended activities of standing or walking or overweight conditions like pregnancy or any other activity that can increase the pressure in the veins of the lower body and legs

Who is commonly affected by Varicose Veins?

People who stand for long durations dentists, doctors who stand for a long time, traffic policemen, soldiers, teachers, security persons, salespersons in shops, drivers, dancers, housemaids, chefs, etc. are prone to Varicose Veins apart from pregnant women, elderly, people with associated diseases and people with a family history. Varicose Veins are also seen associated with conditions like eczema, portal hypertension, piles, and varicocele

How are Varicose Veins caused?

Veins work against gravity to push the blood up to the heart and operate with the help of valves. When these valves in the legs become weak or damaged valves can lead to varicose veins. Valves in the veins and the elastic walls assisted by muscle contractions pump deoxygenated blood to the heart. Once the blood is pumped up, the valves close to prevent backflow. If this process is affected or if the valves are weakened or damaged, blood may flow backward and get pooled in the veins, causing the veins to stretch or twist into varicose veins

What are the symptoms?

During the initial stages, signs and symptoms of varicose veins are,

  • Dark purple or blue veins
  • Twisted and bulging veins that often appear like cords on the legs
  • Mild pain of the legs or tiredness on long-standing

When the disease progresses, signs and symptoms of varicose veins are,

  • Dull ache and /or heaviness in the legs
  • Burning, throbbing pain, muscle cramping, and swelling in the lower legs
  • Worsened pain after sitting or standing for a long time
  • Itching around one or more of the veins
  • Changes in skin color around a varicose vein

What are the complications of Varicose veins?

Complications of varicose veins are,

  • Varicose Ulcers: Painful ulcers near varicose veins, particularly near the ankles which begin as a discolored spot on the skin and progress to a chronic ulcer
  • Blood clots: Due to the continued varicosity clots may form in the deeper veins and lead to severe  leg pain and swelling
  • Bleeding: Veins close to the skin burst to cause bleeding that is usually seen as spurts if the burst happens in a large vein

What is the Ayurveda view of Varicose Veins?

Ayurveda textbooks describe the condition as Sirajagranthi. The causes attributed are intake of swelling causing food like curd, deep-fried food, and red meat, excessive strenuous work, weightlifting, long walks, and long duration of standing. Due to these causes, Pitta gets vitiated and in turn, causes an imbalance in the state of Kapha and Vata in the veins resulting in the disturbing venous circulation and thereby stagnation of blood in the veins.  The symptoms described are aching pain in the legs, numbness, heaviness, tiredness, tortuous veins, and discoloration of the skin

Are Varicose Veins curable?

For prognosis, Varicose Veins are categorized into 3 stages,

  • Stage 1: Varicose veins with distended veins and mild pain and swelling: The condition can be treated with internal medications, exercises, and lifestyle changes. The condition may not recur if proper preventive measures are strictly followed
  • Stage 2: Veins are prominent, swollen, legs have edema, and pain is severe and constant: The condition is treatable and the complications are reduced and the patient can live without discomfort. The treatment involves internal medications, Panchakarma, and bloodletting procedures like Raktamokshana, Jalookacharana, and Siravyadha> Regular follow up and proper preventive measures are to be strictly followed
  • Stage 3: This stage involves complications like ulcers, blood clots, and bleeding episodes. The stage is difficult, yet can be managed through a strict treatment protocol that includes in-house Panchakarma, internal medications, supportive therapies, and very importantly special bloodletting and ulcer care treatments. Modern therapies like stripping and laser treatment can be opted at this stage and followed up with Ayurveda treatments for better prognosis and prevention

What are the Panchakarma therapies for Varicose Veins?

Panchakarma Procedures include,

  • Veshtana: Bandaging with a specialized technique
  • Udvartana: Reverse Massage to improve venous circulation and reduce edema
  • Lepam: External applications

  • Modified form of Uttanapadasana where legs are raised and held on various angles is recommended at least 10 times daily on each leg

  • Maintain optimum weight
  • Avoid standing or walking for long hours
  • Rest the legs at appropriate intervals
  • Avoid sitting with legs dangling or suspended without being rested on a firm surface
  • Avoiding high heels and tight-fitting legwear and footwear
  • Change between sitting and standing positions every 30 minutes
  • Raise your legs and place them on a higher surface while sitting down
  • Eat a balanced high-fiber, low-salt healthy fat diet

What is a brain stroke or Paralysis?

A brain stroke and the resultant paralysis occur when the blood supply to part of the brain is interrupted or reduced, depriving the brain tissue of oxygen and nutrients. This deprivation makes the brain cells die in minutes. A stroke is a medical emergency, and hence immediate medical attention is essential to reduce brain damage and other complications. Integrated Ayurveda management of stroke disability and paralysis is found to be very effective to prevent disability from stroke and also rehabilitate disabled stroke patients

What is stroke or paralysis called in Ayurveda?

Stroke is comparable in cause, symptoms and management to the disease Pakshaghata mentioned in Ayurveda and is stated to be a condition with minimum prognosis. However, with the right treatment given at the right time stroke can be effectively managed with minimum non-significant complications. Stroke or Pakshaghata is caused by the pathology in Vatadosha in executing two of its major responsibilities of circulation and neurological activities. Pakshaghata is described in all Ayurveda classics, among which the noteworthy literature is available in the 28th Chapter of Charakasamhita Chikitsasthana.

What are the symptoms of a brain stroke?

  • Difficulty in speaking and understanding, slurred speech and confusion
  • Numbness, Weakness or Paralysis of the face, arm or leg or one side or both sides of the body or all limbs
  • A sudden, severe headache, accompanied by vomiting, dizziness or altered consciousness, may be indicative of stroke
  • Imbalanced walking or stumbling or sudden dizziness or loss of coordination
  • Vision problems in one or both eyes like sudden blurred or blackened vision or double vision in one or both eyes

How to Self-Diagnosis Stroke? When to see the doctor?

Follow FAST Protocol and perform SMILE Stroke Test

  • Face: Try to smile and see if one side of the mouth is drooping it could be a stroke
  • Try to raise both your arms over the head at the same time. If one arm or both begin to fall, it could be a stroke
  • Speech: Try to speak a simple phrase. If the speech is slurred or strange it could be a stroke
  • Time: If any of the above 3 are noticed seek immediate medical attention

What are the causes and types of a stroke?

Two main causes are ischemia and hemorrhage. The third cause is TIA

  1. Blocked artery causing ischemic stroke: Most common caused by fatty deposits or blood clots or other debris in blood vessels
  2. Leaking or bursting blood vessel causing hemorrhagic stroke: caused by uncontrolled high blood pressure, overuse of blood thinners, aneurysms, trauma, weakness in the blood vessel walls, ischemic stroke, rupture of thin-walled blood vessels, etc.
  3. TIA-Mini stroke: There is another less severe condition transient ischemic attack-TIA caused by a temporary disruption of blood flow to the brain

What are the risk factors of stroke?

  • Lifestyle risk factors like overweight, obesity, sedentary lifestyle, heavy or binge drinking, narcotic usage, use of birth control pills or hormone therapies etc. cause stroke
  • Medical risk factors like CoVid 19 complications, high blood pressure, first or second hand nicotine smoke exposure, high cholesterol, diabetes, obstructive sleep apnea, cardiovascular diseases such as heart failure, heart defects, heart infection or irregular heart rhythm, etc. and personal or family history of stroke, heart attack or transient ischemic attack cause stroke
  • General risk factors like age over 55, male gender, the proneness of race or ethnicity may cause stroke

What are the complications of a brain stroke?

  • Paralysis or loss of muscle movement on one side of the body, or loss of control of certain muscles, bladder control, bowel control, drooping and wasting of one side of the face or one arm.
  • Difficulty in talking, eating, swallowing, language usage, including speaking, understanding, reading, or writing.
  • Memory loss, thinking problems, reasoning issues, judging confusions, or difficulty in understanding concepts.
  • Emotional problems including emotional outbursts, irritability, and depression
  • Pain, numbness, or other unusual sensations like an uncomfortable tingling sensation
  • Changes in behavior, loss of self-care ability, and hence increased dependence on daily activities
